Responses, PDFs, and AI Clinical Summaries
When a patient submits a form, EasyDocForms stores the response and makes it available for staff review. Responses can be reviewed in the dashboard, opened in detail, exported as PDFs, linked to patient profiles, and summarized when AI settings allow it.
Response Dashboard
The dashboard shows recent patient submissions. Staff can:
- Search responses.
- Filter by form.
- Open a response detail page.
- Export a PDF.
- Link or open a patient profile when patient profiles are enabled.
Response Detail Page
The response detail page shows:
- Patient name or anonymous status.
- Submitted date.
- Form title.
- Response status.
- Patient demographic data.
- Form answers.
- Packet source forms when the response came from a packet.
- Reviewer notes.
- Patient profile linking actions.
- PDF export actions.
- Physician summary actions when configured.
Response Snapshots
EasyDocForms can use the form definition snapshot captured at submission time. This helps keep old responses understandable even if the form is edited later.
This matters for:
- Consent records.
- Medicare ABN forms.
- Medical history changes.
- Intake form revisions.
- Audit or migration exports.
Export Response PDF
Use Export PDF to generate a patient response PDF. PDFs are useful for:
- Uploading to an EHR.
- Provider review.
- Patient record archives.
- Case files.
- Consent records.
- Migration exports.
Before relying on a PDF workflow, submit test responses for key form types, especially body diagrams, consent documents, insurance capture, ABN, and matrix-heavy forms.
Physician Summary PDF
A form can have physician summary settings. When enabled, selected fields appear in a compact physician summary PDF.
Use physician summaries for:
- Long intake forms where the provider needs a quick pre-visit overview.
- Pain or symptom forms with many answers.
- Specialty workflows where only selected fields should appear in a clinical summary.
- Appointment preparation.
The summary PDF is generated from configured fields. If a field is missing, check the form's summary settings.
AI Clinical Summary
AI clinical summary can create a concise narrative from patient intake answers. Administrators can enable it in AI Settings and provide organization-level instructions.
Use AI summaries as a review aid. A clinician or trained staff member should review the patient response and the generated summary before using it in clinical documentation, care decisions, coding, billing, or patient communication.
Reviewer Notes
Reviewer notes let staff add internal review context to a response. Common uses:
- Clarify a follow-up question.
- Flag incomplete information.
- Note that a patient profile was linked.
- Record that the response was reviewed before the appointment.
Avoid placing unnecessary sensitive information in informal reviewer notes when it belongs in the clinical record or another controlled workflow.
Patient Profile Linking
When patient profiles are enabled, EasyDocForms can help connect a response to a patient record. The system may show:
- Already linked.
- Create profile.
- Review possible match.
- Incomplete patient data.
- Unlinked.
Linking responses helps create a durable patient timeline that includes intake forms and daily notes.
Packet Responses
If a patient completed a packet, the response detail can show which source forms were included. This helps staff understand the sections that make up a long intake packet.
