Getting Started with EasyDocForms
EasyDocForms helps clinics turn patient paperwork into digital workflows. A typical practice starts by setting up the clinic profile, accepting the Business Associate Agreement, creating or converting forms, sending patient links, and reviewing responses before the visit.
Who This Guide Is For
Use this page if you are setting up EasyDocForms for:
- Patient intake forms for a medical, chiropractic, med spa, wellness, rehabilitation, GLP-1, concierge, or specialty clinic.
- HIPAA-aware online forms that may collect Protected Health Information.
- Consent documents and e-signatures.
- PDF-to-webform conversion.
- Appointment-linked intake with native scheduling or Square Appointments.
- Lightweight charting, daily notes, and provider mobile workflows.
Account Setup Checklist
- Create or log in to your EasyDocForms account at form.easydocforms.com.
- Confirm the organization name and clinic details under Settings.
- Accept the Business Associate Agreement when prompted.
- Add the clinic address, phone, website, NPI, logo, and brand colors if you want them reflected in patient-facing forms and generated PDFs.
- Choose which product workflows are enabled under Feature Access.
- Create your first form manually or use PDF to Webform AI.
- Generate a share link or connect a scheduling workflow.
- Review submitted responses and export PDFs from the dashboard.
BAA Acceptance
Healthcare organizations that use EasyDocForms to create, receive, maintain, or transmit PHI should complete the BAA flow before using the app for live patient data.
In the app, users may be routed to BAA Agreement before accessing the main workspace. This creates a clear setup step before PHI-heavy workflows begin. For more detail, see HIPAA Compliance and BAA.
Feature Access
EasyDocForms can expose different modules depending on the organization:

Main Workflows
Build Forms
You can create forms from scratch, convert a PDF, clone a shared template, or combine existing forms into a packet. Start with Create and Manage Forms.
Send Forms
The live patient delivery method is a secure share link. Share links can expire, limit the number of responses, and optionally require a password. Email, SMS, and QR-code delivery are planned workflow areas, but SMS delivery should be treated as coming soon unless your account has a custom enabled workflow.
Review Responses
Submitted responses appear in the dashboard and response views. Staff can review answers, export the response PDF, generate physician summary PDFs when configured, and link the response to a patient profile.
Use Scheduling
With scheduling enabled, you can configure visit types, availability, a public booking slug, and required forms for each service. A patient can book online and then complete the required intake before the visit.
Document Visits
With clinical documentation enabled, providers can use note profiles and daily notes. The provider app also supports medical AI speech-to-text using a custom medical model so clinicians can dictate note content and review it before saving.
What To Configure First
For a fast launch, configure in this order:
- Clinic Settings - practice identity, contact information, logo, and colors.
- BAA - complete the Business Associate Agreement workflow.
- Forms - create or convert the forms patients must complete.
- Share Links - create the links used for intake.
- Responses - verify PDFs and review workflow.
- Scheduling or Square - attach the right forms to visit types or Square services.
- Team Access - add users and assign only the access they need.
- Data Exports - understand how records can be exported before major migrations or cancellation.
